Description : Description The purpose of this Request for Proposals (RFP) is to select qualified Driver Rehabilitation Professionals, Driver Rehabilitation Specialists, Certified Driver Rehabilitation Specialists or Occupational Therapist Driver Rehabilitation Specialists to provide the following services insupport of eligible Iowa Vocational Rehabilitation Services Job Candidates in the no-tech, low-tech, mid-tech, and high-tech ranges of adaptive equipment needs:1. Individualized driving needs assessment and written report2. Evaluation and written vehicle modification recommendation for adaptive driving needs3. Standard or modified private driver training using the equipment recommended and required by the written assessment and progress reports4. Provision of services at the Job Candidate-s current residential location within Iowa5. Coordination of licensure for Job Candidates under the age of 21 - Documents :
