CITY OF FORNEY REQUEST FOR PROPOSALS (RFP 2025-004)The City of Forney is seeking sealed proposals from experienced and qualified Proposers to submit proposals to provide Advanced Life Support (“ALS”) and/or Mobile Intensive Care Unit (“MICU”) emergency ambulance services for the City of Forney.The outcome of this RFP will be the selection of a Proposer who will become the Contractor with whom the City will negotiate an exclusive, performance-based agreement (Agreement). The initial Contract period will be five (5) years.Proposals should be addressed to the City of Forney and delivered to the Finance Department, located at 402 N. Bois d-Arc, Forney, Texas, 75126. Proposals will be accepted until October 27, 2025, at 3:00 PM.Proposals may be mailed to the City of Forney at P.O. Box 826, Forney, TX 75126, or hand-delivered to the Finance Department at 402 N. Bois d-Arc.
