VA Salt Lake City Health Care System 500 Foothill Drive, Salt Lake City, UT 84148 Project Name: Update Privacy Curtain Track Project Number: 660-23-113 STATEMENT OF WORK SCOPE OF WORK: Furnish all necessary professional services, equipment, labor, materials, supervision, and tools to construct the Project #660-23-113, Update Privacy Curtain Track, located at 500 Foothill Drive, Salt Lake City, UT 84148. The scope of work includes, but is not limited to: Site investigation. Professional installation of new PRVC curtain track system in each room outlined in [Attachment 6 660-23-113 Privacy Curtain Location List]. (Some rooms in this list are dual patient rooms. Site verification needed) Removal of old curtain track system for each room that will receive the new PRVC curtain track system Penetrations from old curtain track system should either have ceiling grid replaced or caulked at the determination of the COR. Maintenance/warranty services for the newly installed systems. Additional 750 count Privacy/Cubicle Curtains needs to be purchased with trolley system track detail and swing arm detail. The Veterans Health Administration (VHA) has a requirement for privacy/cubicle curtains including replacement panels with built in mesh to match existing stock on the VA Salt Lake City campus that meet the VHA IDIQ specifications and high-quality threshold while integrating excellent environmental stewardship. Healthcare furnishings for this procurement include procurement of 750 privacy/cubicle curtains is needed with the current procured number of 836 by the VASLCHCS, (fabric must match currently contracted curtains [Brand-- Arc|Com: Name--Aquifer-x: Number--AC-33403X: Color-- Sand #4]). Installation of Trolley system track detail with a swing arm detail where patient lifts are installed.
