This is a bridging action to continue Accelerating Care Transformation support, which represents a comprehensive and systematic approach to providing personalized care to meet the health and healthcare needs of our patient population anytime, anywhere, and always. Shifting the paradigm of care from a reactive and disease-centered model to a proactive and holistic approach that emphasizes the importance of self-care in the larger context of well-being. It also inherently embraces all aspects of a person-s life - physical, mental, emotional, spiritual, relational, social, occupational, and financial - as equal contributors to achieving a state of well-being and aims to address the social determinants of health by recognizing and addressing the significant influences our environment has on our ability to “be well.” This goes beyond simply offering health and health care services but also includes partnering with local communities to enhance living conditions, promote childhood education, promote health activities, and empower individuals to be advocates for their own health and the health of their loved ones.
The purpose of this bridge action to extend current Accelerating Care Transformation support while the customer is working on the requirement package for the follow-on contract.
