Description:
HSA-s Children and Family Services (CFS) programs protect the welfare of children, improve the lifelong stability of children and youth, and improve the health and strength of families. CFS helps families understand and solve the issues that lead to child neglect, abuse, or exploitation, and includes services in adoption, foster care, childcare, foster youth transition support, and child protection services. CFS is committed to providing foster youth with resources necessary to overcome barriers to achieve self-sufficiency, permanency, and lifelong connections, such as access to safe, affordable, and supportive housing, as well as financial and educational tools.This Request for Proposals (RFP) seeks a provider of the Aftercare program support services designed to provide case management and referral services to community-based providers to eligible foster youth. The anticipated duration of the agreement is 36 months tentatively to begin July 1, 2026 to June 30, 2029. The County shall have the option to extend the term for a period of up to an additional 24 months for a total of 60 months.
