- Alachua County Board of County Commissioners is seeking applications from governmental and non-profit entities for the provision of Project Title.The following apply to this request for application: Instruction to Proposers, Terms and Conditions, Insurance, Scope of Work, Sample Agreement/Contract, Proposed Requirements and Organization, Request for Proposal Selection Procedures, Evaluation Phases, Attachments, and Submittals.CHOICES is a health services grant program offered by the Alachua County Board of County Commissioners (BoCC). It is intended to help indigent, uninsured residents by working in partnership with health care providers in Alachua County to ensure the stable provision of quality health care services.CHOICES was originally created by the BoCC and supported by the voters of Alachua County. The enabling state legislation, Florida Statute Chapter 212, allows counties to levy a local surtax to fund local health care needs. This special sales tax was approved by voters in 2004 for a 7-year period and paid for health care services for eligible citizens. The surtax expired at the end of 2011. Use of remaining sales tax proceeds was subsequently authorized by the BoCC for the creation of a CHOICES Trust Fund. Trust proceeds were anticipated to generate available income to help meet targeted health care needs in our community for an approximate, 10-year period. This Trust- generated revenue also provides for the funding of this competitive health services grant award program also referred to as “CHOICES” and administered by Alachua County-s Department of Community Support Services. Specific details regarding the use of surtax proceeds are set forth in a surtax use plan approved annually by the BoCC.The anticipated, total amount of funding available annually for allocation by the Board of County Commissioners for FY26 and FY27 is projected to be approximately $465,000.00.
