Description:
The Department of Public Health (“City”) issues this Request for Qualifications (RFQ) to establish a Prequalified Pool (PQP) of Proposers for As-Needed Technical Assistance and Consulting Services for the San Francisco Health Network (SFHN), Behavioral Health Services (BHS), and Population Health Division (PHD). For this Solicitation, the City will create four (4) PQPs, one for each of the four service categories listed below. Proposers may apply to be included in one or more PQPs, based upon the Proposer-s response to the Minimum Qualifications and Customer Certification Forms (contained in Attachment A-1). Category 1: Public Health Grant and Research Development. This includes, but is not limited to, identifying Public Health funding opportunities, applying for funding and writing associated grant applications, planning and consulting on Public Health research projects on topics including behavioral health, community health programs, sexual health, and chronic disease prevention, including the responsibility to achieve the City-s grant and research development goals. Category 2: Public Health Organizational Development, Technical Assistance and Training. This includes, but is not limited to, consulting on and developing training materials, leading trainings, acting as a subject matter expert(s) and providing technical assistance to help public health workers build skills, engage with communities with a goal of strengthening all aspects of the City-s public health system, including the responsibility to ensure that all such public health workers remain up to date on all associated state, federal and local rules and regulations. Category 3: Public Health Program Evaluation and Community Needs Assessment. This includes, but is not limited to, evaluating the effectiveness of the City-s public health programs, conducting assessments to identify community needs, collecting and analyzing data, and providing recommendations and reports to help the City make...
