(1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the onlysolicitation. Proposals are requested and a written solicitation will not be issued.(2) This solicitation/synopsis reference number is W50S6S25Q0019 and is being issued as a Request for Quote(RFQ).(3) This solicitation document and incorporated provisions and clauses are those in effect through Federal AcquisitionCircular 2025-05, Dated 07 Aug 2025.(4) This acquisition is not set-aside. The associated NAICS code is 621340 with a $12.5M size standard.(5) The Arizona Air National Guard intends to establish one (1) or more Blanket Purchase Agreements (BPAs) withqualified entities, capable of providing Audiology services on-site at selected vendor-s facility. Any resultant BPA(s)will be valid for a period up to five (5) years unless capacity is exhausted sooner. There is no minimum or maximumguaranteed quantity.Please note that a BPA is not a contract. A BPA is a streamlined method for acquiring products or services on an asneededbasis from qualified sources that are readily available and thus eliminate the need for repetitive costly reprocurements.The Government does not intend to pay for any information provided under this notice.
