RFQ 75H71325Q00011 is hereby amended to add Clause 52.237-7 Indemnification and Medical Liability Insurance and ATTACHMENT 6 - QA. The deadline for the receipt of quotes is hereby extended by 7 days to January 30, 2024.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71325Q00011 has been issued as a Request For Quotation for a firm fixed price commercial services contract for Behavioral Health Nonpersonal Services. See the attached Performance Work Statement for a description of the services to be provided. The performance period is one year, to start within 30 days of the date of award. The performance period includes a one-year base period, and four (4) one-year option periods. The place of performance is Western Oregon Service Unit (Chemawa Indian Health Center), 3750 Chemawa Road NE, Salem, Oregon 97305.
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-07 dated 08/29/2024. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price and past performance. Offerors are advised to submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer.
