1.0 BACKGROUND
The Rosebud Comprehensive Health Care Facility plans to solicit a Firm-Fixed Price, Commercial Item Type Contract to provide Mathews & Parlo LXP100 Color LP06 LVP flooring for the Rosebud Comprehensive Health Care Facility, Box 550, Rosebud, South Dakota.
2.0 OBJECTIVE
The primary objective is to obtain Mathews & Parlo LXP100 Color LP06 LVP flooring for the Rosebud Comprehensive Health Care Facility.
3.0 SCOPE
Flooring being requested is Matthews and Parlo XLP100 LVP flooring and this flooring is what we currently have in our Hospital and Housing units and this is being requested as it is the same color and it all clicks together and we will have pieces if need to replace a damaged floor. Purchasing the exact match would prevent the need to stock several different brands of flooring and prevent the need to replace trim as the trim currently matches the flooring.
4.0 CONTRACT REQUIREMENTS AND PERSONNEL QUALIFICATIONS
The primary objective is to obtain Mathews & Parlo LXP100 Color LP06 LVP flooring for the Rosebud Comprehensive Health Care Facility.
5.0 TYPE OF ORDER
This is a Firm-Fixed Price Purchase Order.
