Description:
As requested by the Human Resources Division (hereinafter, the “Division or Department”), the Collier County Board of County Commissioners Procurement Services Division (hereinafter, “County”) has issued this Request for Proposal (hereinafter, “RFP”) with the intent of obtaining proposals from interested, capable, and qualified vendors to provide health center/medical clinic and wellness services to employees and dependents of the County-s health plan and to provide biometric measurement and laboratory testing services in support of the wellness-based incentive program for covered employees and their spouses, in accordance with the terms, conditions, and specifications stated or attached. The vendor, at a minimum, must achieve the requirements of the Specifications or Scope of Services stated.Historically, the County has spent approximately $850,000 annually to provide on-site clinic services to County staff and their covered dependents and has spent approximately $254,250 annually on biometric and laboratory testing services; however, this may not be indicative of future expenditures for these services.
