STATEMENT REQIREMENT1. INTRODUCTIONThis requirement is to provide the Veterans Health Administration, Wichita Facilities Biomed with all labor, personnel, equipment, tools, materials, parts, and services necessary to repair a Logiq E10 Site ID 530925US8.2. BACKGROUNDThe Wichita Robert J. Dole Veterans Health Administration Facilities has Logiq E10 ultrasound unit that will not pass the loading screen and requires repair. The original equipment manufacturer has already been out to trouble shoot the issue and determined the fix was the Eagle Carrier Board.3. SCOPE OF WORKWORK TO BE PERFORMED: Vendor will provide all labor, personnel, equipment, tools, materials, parts, and services necessary to repair a Logiq E10 Site ID 530925US8. Vendor will put the unit back in working order including reloading software if necessary.HOURS: The regular work hours for this facility are Monday through Friday, from 8:00 am to 4:30 pm (CST), excluding Federal Holidays. All maintenance shall be provided during the regular business hours of this facility. If work is required after hours prior approved will be made by department.RESPONSE TIME: Service needs to be completed by 3/30/2025.PREVENTIVE MAINTENANCE INSPECTIONS: None required.SERVICE QUALITY: All services provided in this contract must meet manufacturers- performance and technical specifications, Federal Regulations, VA Regulations, and meet the requirements of The Joint Commission and Life Safety Code.NONCONTRACT CHARGES: The vendor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer.4.
