3.PUR-466 (Rev 1.) - Proposal Certification - all applicable sections completed and form signed.
4. A separate document, or red-lined document, of all exceptions to FRA LLC General Terms and Conditions for Commercial Products and Commercial Services (06.2022). Exceptions to FRA Terms and Conditions shall be presented with RFP submittal.
5. Finally, we ask that you provide us with the NAICS code that applies to your company/this order.
