Purposethe Intent Of This Funding Opportunity Is To Award Entities To Focus On Specific Targeted Partnerships And Innovative Strategies To Reduce Falls And/Or Falls Risk Among The Majority Being Older Adults (60 Years And Older) And Adults With Disabilities, With A Focus On Those With Greatest Social And/Or Greatest Economic Needs (I.E., Target Population For This Funding) Through The Implementation Of Evidence-Based Falls Prevention Programs With An Innovative Person-Centered Approach.This Funding Opportunity Has Two Goals:Goal 1: Develop An Innovative Fall Prevention Method That Includes Targeted Partnerships And Outreach To Recruit The Target Population Into Small Group In-Person Evidence-Based Falls Prevention Programs.Goal 2: Design An Evaluation Of Your Intervention To Assess The Impact Of The Effort On Reducing Falls And/Or The Risk Of Falls, And Develop And Disseminate 508-Compliant Program Results, Resources, Lessons Learned, And Process Documents To Inform Practice, Service Delivery, Program Development And/Or Policymaking For Other Organizations To Replicate This Approach, To Include A Comprehensive List Of Partnerships Developed.The Administration On Aging (Aoa), Part Of The Administration For Community Living (Acl), Plans To Award 2 Cooperative Agreements With A Three-Year Project Period, Subject To Availability Of Funds.
