Efforts Will Include Ensuring Collaboration With Partners, Creating And Disseminating Resources And Education, And Capturing Best Practices And Learning. Additionally, The Entities Will Design An Evaluation To Assess The Impact Of Efforts On Reducing Falls And/Or The Risk Of Falls.
