- Through this Solicitation of Quotes, the Mississippi State Department of Health Heart Disease and Stroke Prevention Program (HDSPP), Mississippi Delta Health Collaborative Program (MDHC) and Diabetes Prevention and Control Program (DPCP) will contract a facilitator to support the establishment and leadership of the Mississippi Alliance for Cardiovascular Health (MACH) and the Diabetes Advisory Council. The facilitator will provide leadership, coordination, and learning collaborative facilitation to build and sustain these advisory networks, guide coalition-building efforts, and promote patient-centered strategies to improve cardiovascular health and diabetes outcomes across Mississippi. The contract award will be for an amount up to, but not to exceed, $75,000.
