Amendment 00003:
The purpose of this Amendment 0003 is to:
Incorporate the revised solicitation 80LARC25Q7001, which removes the Licensed/Certified Fitness Director as key personnel (Section A.2.19), fitness center as installation-accountable government property (Section A.2.20(c)(ii)(7)), TPA 4: Contractor-s experience in promoting and educating audiences on physical and mental health, fitness, regular physical exercise, and screenings for early detection of diseases (Section C.2.10(d)(11)) and PWS 4.0 from the Pertinence/Relevance Table. Incorporate an updated Exhibit A - PWS, which removes the requirement to operate the Physical Fitness Program and Fitness Center. The requirement for the Employee Assistance Program (EAP) has been updated to 24 hours per day, seven days a week by telephone or virtual appointments instead of on-site and off-site EAP Counseling Service. Incorporate an updated Exhibit C - Installation-Accountable Government Property, which removes the Fitness Center Equipment. Incorporate an updated Exhibit D - Position Descriptions, which removes the Fitness Center Director and Fitness Specialist/Trainer positions. Incorporate an updated Attachment 1 - Pricing Form, which removes the Mandatory Fee-For-Service for All Employees options related to Physical Fitness Program and Fitness Center requirement. Update the quote due date stated in Block 8 on the SF1449 from 05/20/2025 to 05/30/2025._____________________________________________________________________________
