The Indian Health Service, Navajo Area, Gallup Service Unit, Gallup Indian Medical Center located at 516 E. Nizhoni Boulevard in Gallup, NM 87301 intends to negotiate a single-sourced award with Tocabe Indigenous Marketplace Inc. (Tocabe), UEI: FAWTPJZA2KM7, of Denver, Colorado for the development, preparation, and delivery of medically-tailored meals for heart-failure patients within the local communities served by the Gallup Service Unit. The medically-tailored meals must utilize Navajo traditional recipes and incorporate locally-sourced produce and meat from Navajo/Native farmers. The meals must be specifically designed by a registered Dietitian to meet unique dietary and health needs of patients diagnosed with heart failure diseases. The requirement-s period of performance is estimated at four to six months from time of award, which may be extended under 52.217-8 Option to Extend Services for an additional but no more than six months after the base period of performance.
