1. Scope of Work
This requirement needs to provide lead aprons, thyroid shields, protective eyewear, and apron racks, with many items requiring custom embroidery and specific staff name tags.
2. Place of Performance
The product is requested to be delivered to Surgery Service, building 100, MOR.
10,000 Bay Pines Blvd, Bay Pines, Florida 33744, via the Warehouse Building 36 for Equipment.
3. Period of Performance
The product is requested to be delivered no later than December 2nd.

Salient Characteristics
General Requirements for Lead protection eyewear:
0.75 mm Pb equivalence or greater
Must have anti-reflective and anti-fog included
Outside engraving with staff name or Operating Room
Must provide Inservice for sizing to ensure correct fit
Must be certified to ANSI Z87.1 and ASTM radiation protective eyewear standards
Must include engraved or permanent labeling indicating Pb equivalency, manufacture and compliance with safety standards
