Preventative Maintenance Services for Bayer MedRad Injector STX VA Health Care VA Medical Center IAW SOW.
Base Period: 12 months ADC
Option Year 1 (OY1): 12 months
Option Year 2 (OY2): 12 months
Option Year 3 (OY3): 12 months
Option Year 4 (OY4): 12 months
The following are the instructions for interested vendors:
(1) Each quote should provide the certification on VAAR 852.219-75 VA NOTICE OF
LIMITATIONS ON SUBCONTRACTING—CERTIFICATE OF COMPLIANCE FOR
SERVICES AND CONSTRUCTION (JAN 2023) (DEVIATION).
(2) Sign blocks 30a-30c on the first page of the SF1449 and fill in the quoted price on B.2,
Price Schedule, for each Item Number and add the Grand Total.
(3) To be Technically Acceptable: Provide sufficient evidence that demonstrates your
company is certified and qualified to service the MEDRAD Equipment. Examples of
Evidence: Technician Training Credentials & Technical Competencies. Service History &
Performance Records. Certifications Held by the Company.
The lowest technically acceptable quoted price will be awarded the contract for the full-service
contract for Preventive Maintenance Services for the Bayer MedRad Injector.
