SF1449 must be completed as follows:

PAGE 1: Block 12, 17a, and *24 (*Combined Overall Total for the entire Contract)
PAGE 1: Block 30 a / b / c (***must be SIGNED/DATED; can't accept quote w/o signature***)
PAGE 3 : Unit Price, Overall Amount for each individual Line Item (a total of 1)
PAGE 36 through 48: Clause 52.212-3 (fill in applicable slots)
PAGE 48: Clause 52.204-26 (fill in applicable slots)
RFQ is considered "completed".

Completion and submittal of SF1449 is mandatory in order for your quote to be considered (no exceptions).

NOTE 1: This solicitation is Full and Open Competition under NAICS Code 611430.

NOTE 2: Your quote must reflect the EXACT specifications and details listed on the SF1449 (no exceptions). Failure to meet this requirement will be considered as "Non-Responsive" and you will not be considered for this solicitation. SF1449 must be completed and attached with any quoted offers. Please ensure to provide sample and shipment tracking number. Read Addendum (PAGE 34) incorporated in the solicitation document for details. Schedules included as a separate document.
