Description:
The Minnesota Department of Information Technology Services (“MNIT”), partnering with the Minnesota Department of Health (“MDH”), Children and Youth with Special Health Needs (“CYSHN”) section, is seeking proposals for an effective, integrated developmental screening data collection and management solution that will support the needs of MDH-s Follow Along Program. The system must use the Ages & Stages Questionnaires®, Third Edition (ASQ®-3) and the Ages & Stages Questionnaires®: Social-Emotional, Second Edition (ASQ®:SE-2), which must be licensed from the intellectual property owner (Brookes Publishing) and programmed with fidelity to the guidelines for administration outlined in the ASQ®-3 and ASQ®:SE-2 User-s Guides. The scope of this procurement includes both the provision of the software solution and the performance of implementation related services. The State may also engage the vendor to perform additional work on the implemented system (such as enhancements, implementation of new modules, etc.) via subsequent amendment(s) to the resultant contract. The State is seeking an existing Commercial Off-the-Shelf (COTS) solution that is configurable/customizable to meet State needs. The State is NOT seeking a vendor to custom build a solution from the ground up.
