Description
This solicitation is being issued on an unrestricted basis to allow for Small Businsess Set Aside Base plus 3 Option Year Contract. The associated NAICS code is 811210. Offerors must be registered in Systems for Award Management (SAM) at www.sam.gov per FAR 52.212.1. All offers must also have a Unique Entity Identification Number at (SAM) at www.sam.gov and be able to submit invoices through Wide Area Work Flow (WAWF). Lack of registration in SAM will qualify contractor as ineligible for award. All responsible sources may submit a quotation in response to this solicitation which shall be considered. The solicitation is being issued as a Request for Quote (RFQ) for a firm fixed type contract based on the Lowest Price Technically Acceptable. The Naval Medical Research Command (NMRC) intends to award a contract resulting from this solicitation to the responsible offeror. No third party vendors (foreign or domestic) are accepted with NMRC unless otherwise stated.

Statement of Work
A full maintenance service plan needs to be set up for three incubators, one refrigerator, two -20C freezers, and four -80C freezers. Service needs to be performed on these units on at least an annual basis and additionally if problems arise to ensure proper functioning and safety of users for NIDDL and to ensure compliance with the regulation from the College of American Pathologist (CAP) as the accrediting body for NIDDL. In addition, the vendor needs to be able to provide an equivalent substitute loaner freezer if a freezer fails and the service needs to take place off-site. Therefore, the need is for a vendor that can provide technicians need to be qualified to perform service Forma/Thermo Forma incubators, refrigerator, and freezers also LabRepCo freezers, as well as, provide loaner freezers if service of our freezers need to be accomplished off the premise of the Naval Medical Research Command.
