The below addendum is hereby incorporated in the solicitation in full text as follows:ADDENDUM to 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES(a) Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:Include the signed SF 1449, Solicitation/Contract/Order for Commercial Items, with Blocks 12, 17a, 17b, 30a, 30b, and 30c; completed.a. Email your combined single pdf quote submission to david.griganavicius@va.gov at or before the exact time specified in due date of this solicitation.b. Completed Section B.1 Contract Administration, provide Contractor Name SAMs UIE#, Address, point of contact(s) name, phone, and email by the time specified in the Solicitation for receipt of offers. c. Completed B.2 Price/Cost Schedule. Enter your prices under B.2 Price/Cost Schedule. Prices need to include any FOB destination shipping cost.
