STATEMENT OF WORK DENTAL FIXED PROSTHDONTICS LABORATORY SERVICESFixed Dental Prosthetics ServicesOBJECTIVES: An overview of this SOW will include goals and objectives. The Hampton VAMC North Battlefield HCC has a need to continuously utilize dental fixed prosthodontics laboratory services for Veterans requiring special dental lab work.General RequirementsThe contractor shall provide laboratory support including fixed dental prosthodontic laboratory work for Hampton VAMC North Battlefield HCC.Required ServicesContractor shall provide all dental laboratory services materials and supplies necessary to fabricate and complete the following items:Porcelain fused to high noble metal (or equal) crowns sing low abrasive porcelainPorcelain fused to high noble metal (or equal) bridges using low abrasive porcelainCast all metal crowns & bridgesAll ceramic crowns & bridges Procera, Empress, In-ceram, Belle-Glass (or equal)EsthetiClasp (or equal) partial denture -framework, setup, processEsthetiflex (or equal) partial denture -framework, setup, processComfort Zone Plus (or equal) occlusal guardTraditional occlusal guardImplant Custom Abutment (Titanium, Zirconium)Porcelain VeneersAcrylic provisional crowns and bridges per toothMaryland BridgesPrecision attachmentsService Turn-around TimeAll work will be completed within ten (10) working days from the day of arrival via at the contractor s laboratory.The contractor shall return all products to Hampton VAMC North Battlefield HCC, Chesapeake via FEDEX or UPS with the contractor bearing the expense for shipping.Specific RequirementsIn house technicians working within the United States shall perform all removable prosthetic/crown and bridge procedures.Teeth may or may not be provided by the VA.
