STATEMENT OF WORKGENERAL INFORMATION Title of Project: Olympus Endoscopy SupportScope of Work: The vendor will provide support for the Olympus endoscopy equipment, listed below, that is currently in use at the Baltimore VA Medical Center. Support will include:Full repair coverage with no cap for all contracted equipment, including accidental damage.Next day shipping in both directions included at no extra chargePrioritized repairs, with 24 - 48-hour turnaroundAccess to loaner equipment at no extra charge24/7 technical supportOnline repair tracking and historyPeriodic and on-demand training and support provided by trained support specialistsAccess to preventative maintenance and educational coursesService Contracts administrative support during typical business hours All repairs must return the equipment to the device s original or current manufacturer specifications. The vendor will supply the Baltimore VAMC Biomedical Engineering department with a copy of a detailed service report that meets the requirements of 21CFR820, subpart N, within 2 weeks of a repair being completed. All exchanges must be documented with an RMA provided to the Baltimore VAMC Biomedical Engineering department including the serial numbers of both the equipment being replaced and the replacement equipment.
