Description
The Indian Health Service, Tohatchi Health Center, located at 07 Chooshgai Drive, Tohatchi, NM 87325 has an upcoming requirement for Pharmacist Services. The projected Period of Performance will be as follows:
Base - Date of award through six (6) months
1st Option Period - six (6) months.

The North American Industry Classification System or NAICS code is 561320 with a small business size standard of $12.0 M.
The Request for Quotations No. 75H71025Q00084 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 2/7/2025. Evaluation factors may include:
Technical Capability Key Personnel qualifications in accordance with the PWS as described in Attachment A - Performance Work Statement particularly those minimum qualifications identified in Attachment A - III Contractor Minimum Requirements/Qualifications Past Performance. The Government reserves the right to retrieve and review past performance information from the Contractor Performance Assessment Reporting System (CPARS) and other federal sources and systems Price Quotes will be due on or by 2/14/2025. The acquisition is a Small Business set aside.
In accordance with 52.204-7(b)(1) prospective contractors must have - and maintain (in accordance with 52.204-13(c) - an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
