Contractor will provide all labor, equipment, materials, and supervision necessary to remodel the Chapel at the Fort Wayne VAMC. This will include but is not limited to the following.
Remodel the Fort Wayne Chapel. Demolition as stated in the drawings and specifications. Construction as stated in the drawings and specifications. Adherence to all infection control requirements given in the drawings and specifications.
A non-destructive hazardous materials survey was provided in the project package. As such the contractor may encounter other hazardous material (asbestos containing material, lead based paint) during demolition.
All work to be completed at the VA- Northern Indiana Health Care - Fort Wayne Campus, 2121 Lake Ave., Fort Wayne, IN 46805.
Work to be completed within 183 calendar days from notice to proceed.
*8 Jan 2025 - Specifications have been added to list of attachments.
*14 Jan 2025 - Amendment A00001 is hereby issued to establish RFI due date of 21 January 2025 by 2:00 pm EST.
*6 Feb 2025 - Bid due date has been extended to 21 February 2025 at 2:00pm EST. Conference call in information will be provided in a future amendment. RFI responses are still being gathered, and they will be provided in a future amendment. Additional information concerning environmental requirements has been provided, in addition to a copy of the site visit sign-in sheet.
20 Feb 2025 - Amendment A00003 extends bid due date to 7 March 2025 at 2:00pm EST.
5 March 2025 - Amendment A00004 incorporates RFI Responses, along with revised Drawings/Specifications and references to RFI Responses. In addition, the bid due date is being extended to 14 March 2025 at 2:00pm EST. A future amendment will be issued providing the conference call-in information for the bid opening. - Documents:
