City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1170 Short-Term Home Care for Older Adults: Chore, Homemaker, and Personal Care Services The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announce its intent to seek proposals from non-profit organizations to provide Short-term Home Care for Older Adults: Personal Care, Homemaker, and Chore services to eligible individuals experiencing difficulties with activities of daily living (ADLs) at home. Due to the short-term nature of this program, proposers should detail how they intend to define and administer the short-term aspect of this program. Historically, this program has provided a capped amount of services in specific situations. Those have included clients discharged from a hospital or institution, individuals in the process of applying for the State In-Home Supportive Services (IHSS) program, or other unique situations where short-term services are appropriate. DAS Short-Term Home Care for Older Adults program is currently in operation with clients receiving services. In the event that a new grantee is selected for this service, the new grantee needs to work with DAS and the current provider on effective transfer of active clients in order to minimize disruption in services to program participants. Applicants may elect to apply for this grant as a lead organization with collaborative partnership(s). Collaboration that creates a greater impact in service delivery through the sharing of skills, knowledge, and resources is encouraged. Applicants must clearly identify the roles of all partnerships named in the proposal, including the lead organization.
