City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1171 Emergency Residential Care Facility for the Elderly Bed Placement The San Francisco Human Services Agency (SFHSA) announces its intent to seek proposals from Residential Care Facilities for the Elderly, Assisted Living Facilities, Adult Residential Facilities, and Continuing Care Retirement Centers interested in contracting to provide innovative and client-centered emergency placement services to Adult Protective Services (APS) clients: elders aged 65 or older and adults with disabilities over the age of 18 who are experiencing abuse, neglect, exploitation, or self-neglect. The purpose is to promote the safety and well-being of APS clients. The facility must be licensed by California-s Department of Community Care Licensing and located in the City and County of San Francisco. The facilities must provide for individual-s needs and emergency placement services to Adult Protective Services (APS) clients who are elders aged 65 or older and adults with disabilities over the age of 18 who are experiencing abuse, neglect, exploitation, or self-neglect. The emergency placement services will provide access to a safe respite bed, meals, and supportive personal care services on a 24-hour basis. The facility must have experience providing services for people with cognitive impairment, developmental disabilities, and behavioral health needs. Contractor shall have a total of up to three (3) beds available at all times per month (for details, please refer to Attachment 2 - Budget Form). SF-HSA anticipates awarding one contractor as a result of this RFP. The source of funding for these services are federal, state, and local funds. Payment for all services provided in accordance with provisions under this contract shall be contingent upon the availability of funds.
