City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1180 Citywide Nutrition Education for Older Adults and Adults with Disabilities The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announces its intent to seek proposals from nonprofit organizations interested in providing a citywide nutrition education program for older adults and adults with disabilities. The purpose of this program is to deliver evidence-based nutrition education that supports informed food, nutrition, and physical activity choices aimed at improving or maintaining health. The program will also address nutrition-related conditions, promoting lifestyle changes that empower individuals to actively manage their health and wellness. Through this Request for Proposals (RFP), DAS aims to ensure equitable access to high-quality nutrition education that meets the diverse needs of San Francisco-s older adults and adults with disabilities. At least one grant will be awarded. However, DAS may award more than one grant depending on the quantity and quality of proposals submitted. Funding for the program has been allocated based on current service provision, the Dignity Fund Community Needs Assessment, and the Service and Allocation Plan for FY 2023-24 through FY 2026-27. This procurement is for an existing program. An agency(s) awarded a new grant may be required to collaborate with DAS to develop an outreach plan, a transition plan, or both, depending on the needs of the program(s). If a transition plan is necessary, it will be essential to ensure service continuity for current consumers when the new grant period begins. A successful bidder(s) will work closely with DAS to facilitate a smooth transition, minimize any disruptions to service delivery, and, if applicable, conduct effective outreach to engage new and existing participants.
