Amendment 0005
The purpose of Amendment 0005 is to:
a. Revise the SAM.gov combined synopsis and solicitation posting with the following changes:
---Revise the sentence from "The price analysis will take the total overall price of the following: CLIN X001, CLIN X002 (Although Service Order Pricing is capped at $60,793.60 the evaluation for this CLIN will be based on the total of the base and all options PER EACH as addressed on the Pricing schedule sheet.), and CLIN 0003 Phase In/Phase Out." to " The price analysis will take the total overall price of the following: CLIN X001, CLIN X002 (Although Service Order Pricing is capped at $80,000.00 the evaluation for this CLIN will be based on the total of the base and all options PER EACH as addressed on the Pricing schedule sheet.), and CLIN 0003 Phase In/Phase Out.”

b. Revise Attachment 5 - W9124R-25-Q-A0002 - SF 1449 with the following changes:
