Firm Fixed-Price, Non-Personal Health Care Service, Commercial Item Purchase to provide a Physical Therapist for the Woodrow Wilson Keeble Memorial Health Care Center (WWKMHCC), Sisseton, SD; Base Plus Four (4) Option Years.
2.0 OBJECTIVE
The primary objective of this project is to provide one (1) Physical Therapist.
3.0 SCOPE
See attached Performance Work Statement (PWS).
4.0 CONTRACT REQUIREMENTS/ AND PERSONNEL QUALIFICATIONS
The contractor is required to follow all federal, state, and local requirements pertaining to Temporary Help Services.
5.0 TYPE OF ORDER
This is a Firm Fixed-Price (FFP) purchase order.
6.0 ANTICIPATED PERIOD OF PERFORMANCE
Period of Performance: Base plus four (4) option years.
