- Description District 2 has provided funding for an Aftercare program available to District 2 JCS referred Youth through a Decategorization program (DCAT) for the past three years. The DCAT program was limited to providing services to clients returning to a home-like setting. District 2 is seeking a Respondent to provide Transition Services to Youth throughout all of District 2. Preference will be given to Respondents that will provide this service to all District 2 counties; however, proposals for services of all or any combination of the counties in District 2 will be considered and as a result, this RFP may result in more than one contract. The primary goal of the program is to aid Youth including Youth transitioning from out-of-home placement back into the community. District 2 anticipates Respondent will have 3 FTE Transition Coaches to fulfill these services.
