Description : This project is designed to afford Intermediate Units (IUs) across the Commonwealth of Pennsylvania an opportunity to evaluate and enhance transition programming through data-driven decision-making. Over the course of two years, the grantees use a researched based selection process (see Appendix) to screen and assess all local education agencies (LEA)s in their region. One LEA will be selected based upon the highest need according to the score from the assessments. The grantee will collaborate with the selected Local (LEA) to implement evidence-based practices, and foster systems-level changes that significantly improve the transition planning and outcomes for students with disabilities within the selected LEAs School District.
