PURPOSE:
The Albuquerque Area Indian Health Service (AAIHS), Santa Fe Indian Health Center (SFIHC) & Satellite Clinics (specified below) has a requirement for two (2) Independent Social Workers, Santa Fe, NM 87505.
STATEMENT OF WORK:
Social Worker Duties:
The Contractor shall provide Optometry Evaluation and Treatment under a non-personal services agreement as a Licensed Independent Social Worker with the following specifications:
The contractor shall provide Licensed Independent Social Worker non-personal healthcare services with the following specifications:
Contractor shall provide counseling assistance to individuals and families with psychological or social problems and crisis. Consults with Clinical Director, Chief of Staff, and/or Staff Psychiatrist on management or referral of all difficult or complex problems.
Contractor shall provide crisis intervention to include assessments, counseling, diagnosis and coordination of care. May include coordinating admission in psychiatric hospital for acute behavioral health intervention.
Contractor shall provide services for the adolescents and adults, including the geriatric patient. The services provided will meet the unique physical and psychosocial (emotional, spiritual and social-cultural) needs of these patient populations by means of direct care and teaching.
