Description
The Indian Health Service, Navajo Area, Shiprock Service Unit located at U.S. Highway 491 North, Shiprock, NM 87420 intends to negotiate a single source award with Milestone Medical Technologies, Inc. (UEI: X9A2U2JNF257), for a twelve (12) month/yearly service contract plan to cover an UltraSAFE Formulin Dispensing system. The projected period of performance is a total of five (5) years from the award date.
Base Period: twelve (12) months from time of award. Option year-one: approximately from 6/01/2026 to 5/31/2027 Option year-two: approximately from 6/01/2027 to 5/31/2028 Option year-three: approximately form 6/01/2028 to 5/31/2029 Option year-four: approximately from 6/01/2029 to 5/31/2030

This Notice of Intent to Single Source is not a request for quotations and a solicitation will not be issued in conjunction with this Notice of Intent. Any vendor that believes it can provide the stated service contract may provide a capability statement. Verbal responses are not acceptable and will not be considered. Responses to this notice shall be submitted no later than 2:00 PM (MDT) on May 12, 2025 via electronic mail to Dallas Begay, Purchasing Agent at Dallas.Begay@ihs.gov.
